On-Site Service Coordinator for Behavioral Health Housing Units

County Health, Behavioral Health and Recovery Services (BHRS) is looking for a qualified service provider to deliver onsite service coordination for eleven (11) behavioral health housing units at the Monarch at Redwood affordable housing development located at 2400 San Mateo Avenue, Redwood City, CA 94063. BHRS behavioral health units target individuals living with serious mental illness (SMI) and/or substance use disorders (SUD) who are homeless or at risk of homelessness. The Monarch at Redwood development owned by Affirmed Housing California and managed by John Stewart Co. is an 86-unit affordable housing project with 11 units set aside for BHRS eligible tenants. Referrals for behavioral health units will be provided by BHRS. The Behavioral Health Services Act (BHSA) requires BHRS to maximize available Medi-Cal funding for eligible housing-related services. Tenants in the BHRS-designated units may be eligible for CalAIM Community Supports, including Housing Transition Navigation Services, Housing Deposits, Housing Tenancy and Sustaining Services, and Transitional Rent, as available through their Medi-Cal managed care plan. The selected provider will ensure tenants access available Community Supports benefits and billing the managed care plans and BHRS for the appropriate services.
